目前已完成3项Ⅲ期临床试验,即Abatacept in Inadequate Responders to Methotrexate (AIM)、Abatacept Trial in Treatment of Anti-TNF Inadequate Responders (ATTAIN)及Abatacept Study of Safety in Use with other RA Therapies (ASSURE)。 AIM[[15]’[16]’[17]’[18]]试验观察了Abatacept与MTX联用对MTX无效的患者的疗效。652名患者随机分为治疗组(Abatacept 10mg/kg分别于第1天、第15天、第29天静脉给药,之后每28天给药,同时予MTX≥15mg/周)和对照组(安慰剂及MTX≥15mg/周)。结果6个月后两组ACR20%缓解率分别为67.9% 和39.7%, 12个月后ACR20%缓解率分别为73.1%和39.7%,并且对比治疗12个月前后患者手、足的影像学资料发现,联合治疗组的骨质侵蚀分数、关节狭窄分数及总分均较对照组低,分别为0.63和1.14(p=0.029)、0.58和1.18(p=0.009)及1.21和2.32(p=0.012),表明Abatacept对MTX无效的患者不仅可明显改善临床症状,而且可显著抑制关节破坏。
